The design of this sheild tends to pinch while nursing. There is not enough room for the nipple to expand while suckling & my nipple ends up getting pinched in the process which causes extreme pain. I purchased it for the cut out, hoping to have more skin contact with my daugther, but Madela also has a shield with the cut out & it works so much better is much more comfortable for me while nursing. I would not recommend this shield for purchase.

I have small nipples and any other brand just didn't fit right and caused even more pain. These are a perfect fit if you have smaller than average nipple size. They saved me when I was about to give up breastfeeding...

This nipple shield was recommended to me as a way to wean my son off of the medela nipple shield we had been using for months. It is much thiner and softer then the medela nipple shield and has many more - but smaller - holes in the shield for milk to come through. It did not really work for us, but would be a great alternative to try.
